The purpose of this registry is to collect basic contact information and health information from individuals 19 years of age and older who have experienced a concussion or head injury. .

The registry information will be used to identify research opportunities for individuals with concussion/brain injury and you may be invited to participate in research projects related to better understanding the concussion/brain injury.
